The Statewide Health Improvement Partnership (SHIP) creates wellness across Minnesota’s communities through community-driven, evidence-based, and equity-focused programs that tackle the root causes of chronic diseases. Through SHIP, Living Healthy Washington County staff work hand-in-hand with local schools, businesses, and community organizations to create lasting changes that support resident health. 2023 included the launch of our second Healthy Opportunities for People through Equity (HOPE) site in Oakdale, joining our first HOPE site, Landfall. HOPE is a data-driven initiative designed to identify areas within Washington County where residents are experiencing poor health outcomes. We work alongside residents to support community-led solutions to address their health priority areas.
Communication products that are funded by the Statewide Health Improvement Partnership (SHIP) must be approved by the Minnesota Department of Health (MDH) prior to publication to ensure that branding requirements are met and messaging is clear.
