The Community Leadership Team (CLT) is a collaborative which provides guidance for LHWC strategies throughout the county. There are 20-22 active members on the CLT from the north, central and south parts of the county who represent: communities, health care organizations, schools and worksites. The CLT is comprised of key community partners who are committed and actively investing their expertise, leadership, and other resources which are essential to the success and sustainability of LHWC.
In 2021, Living Healthy Washington County continued to improve community health with a deepened focus on our new health equity efforts. Community partner projects increased residents’ access to healthy foods, physical activity, mental well-being, and reduced use and exposure to commercial tobacco products.
Communication products that are funded by the Statewide Health Improvement Partnership (SHIP) must be approved by the Minnesota Department of Health (MDH) prior to publication to ensure that branding requirements are met and messaging is clear.
